All eligible study members were contacted: 2,662 study members gave information, of which 2,474 study members completed a postal questionnaire and 2,229 study members completed a clinic or home visit.

Data collection
A feasibility study on 10% of the NSHD sample living near the Wellcome Trust Clinical Research Facility (CRF) in Manchester took place during 2006-2007. Data from the remaining sample were collected from 2008 to 2010 at CRFs located in Edinburgh, Birmingham, Cardiff and London.

Training for data collection
The study team and specialist colleagues trained the nurses for 1-2 days at each of the CRFs.
