Section 1 – Contact details for applicant and co-applicant.

We require the applicant and co-applicant contact details to process your application and so we know who will need to be given access to the data.
We need to know the institution information as we require an institutional data sharing agreement to be in place before we can share the information. We have asked about the co-applicant institution, in case this is different from the applicants.
Question 9. Further co-applicants. Please include the Name, Position, Institution and Email address for any additional co-applicants.
NOTE: If your application is part of studying for a PhD, your supervisor(s) must be named as co-applicant(s) on the application.
If you have not finalised the project team / co-applicants, this should not stop you from completing the form. Additional users can be added/requested at any point using the Additional Users/Data Request form.
Section 2 – Project information
We publish the list of approved projects, so we can be transparent about how the data are being used. It also means Applicants can check if similar projects have already been approved.

By submitting the form you are consenting to the disclosure of this limited set of information to other researchers. If you do not consent to this, please do not submit the form.
Section 3 – Project funding and internal project description
This information is for internal use only.
We ask about peer reviewed and funding because we want to check if the proposal has already been reviewed and to make sure the applicants have the ability to do the work.
Brief description of the project including proposed output. Please provide a brief description, no more than 1 side of A4 (or 500 words) and no more than 10 key references.
Section 4 – Public summary
This information will be made public and shown alongside the project name and dates of the project from Section 2. Please can this be no more than 150 words and no references.
Section 5 – Speciality datasets
This is for internal use only.
This is to ensure there is the experience and expertise within the team applying for the data to be able to analyse it.
Section 6 – Imaging data
This is for internal use only.
Please let us know if the project involve data collected for Insight 46 or imaging data.
If imaging data is required, please let us know which sequences are required.
We ask about the feasibility and quality control for imaging analysis. These datasets are complex, and we want to make sure there is the experience and expertise within the team applying for the data to be able to analyse it.
Section 7 – Variables required
This is for internal use only.
If you have a list of variable names taken from Skylark please include them. If not, please describe which variables will be required and which years / data collections they come from.
We are checking if there are any variables which are being requested which have the potential to be disclosive or have low cell counts.

We are interested to know how applicants have heard about the NSHD data and use this internally for our metrics.
